NDIS Support Coordination and Behaviour Support

Support coordination empowers young people to coordinate their NDIS supports, helping them implement positive behaviour strategies and develop independence.

Support Coordination

Support coordination helps to facilitate young people’s independence and participation in the community, enabling them to take control of their NDIS supports. Our Coordinators work with participants to access disability and mainstream service providers of their choice to build capacity and develop autonomy in their networks and communities.

Behaviour Support

We work with young people to identify behaviours of concern and design positive behavioural support strategies. Our plans identify behavioural functions and triggers and how to de-escalate these behaviours, reduce behaviour frequency and minimise the use of restrictive interventions. We provide support and training to participants’ families and support networks.

Our Support Coordination team can support you to:

  • Implement the supports in your or your child’s/young person’s NDIS plan
  • Coordinate assessments, reports and service proposals
  • Support you to identify and select providers and monitor plan outcomes & expenditure
  • Strengthen your capacity to self-direct your supports and participate in the community 
  • Identify strategies and solutions for managing risks and sustaining informal supports
  • Access mainstream services and ensuring these services meet their obligations (i.e. housing, education, justice, health)
  • Identify and seek relevant assessments and associated service design for participants with risk behaviours and behaviours of concern

Who is eligible

Children and young people with an approved NDIS support plan which includes line items for coordination of supports and/or behaviour support.
