The Specialist Disability Practitioner Support program aims to address the challenges many vulnerable families experience with accessing disability support needs, particularly those available through the NDIS. The program assists families to navigate systems of disability support, including the NDIS, disability advocates, disability service providers and mainstream services.

Family Services Specialist Disability Practitioners work with family services providers as well as directly with families to offer this service. Practitioners can provide a free consultation and/or short-term case coordination service to help families make the most of their NDIS and disability supports.

The program aims to:

  • Offer disability and NDIS related case consultations to Family Services workers within the Brimbank and Melton region.
  • Offer secondary consultations on disability and NDIS related issues which may include one off advice or short-term joint case coordination
  • Work collaboratively with family services providers to encourage the early identification and early support of families where complex disability support needs are present.
  • Build the ongoing capability of family service practitioners to support vulnerable families with disability to maximise disability supports and outcomes available through NDIS.

Who can access this service?

The program is open to families with a child or parent/carer with a disability residing in the Brimbank or Melton regions. An NDIS plan is not required to access the service.
