The program provides support to vulnerable families with disability and family service practitioners in the Melton/Brimbank local government area.
What we offer
Support to vulnerable families with disability to:
- Access NDIS and/or make the most of their NDIS plans
- Navigate the NDIS and disability supports system.
Guidance is offered to family services practitioners by:
- Providing secondary consultation on disability and NDIS related cases
- Providing short-term case coordination service
- Encouraging the early identification and support of children or parent/carers with disability support needs
- Building the capacity of family service practitioners to work effectively with children or parent/carer with NDIS and disability support needs.
Eligibility
- Vulnerable families with disability and family service practitioners located in Brimbank and Melton local government areas
- An NDIS plan is not required to access the service
- Referred by self or agency.
